I understand that MSMN nor its agent can guarantee that repairs can be finished at the time of the service call. I understand that MSMN nor its agent can guarantee the service call will be covered under the manufacturer's warranty.

MSMN or its agent will submit warranty claims on my behalf if MSMN deems it may be covered under the warranty. I understand and agree that should any manufacturer's warranty compensation be less than my spa service bill that I am responsible to pay the difference. MSMN as a factory authorized service center can determine at the time of the call if things are NOT covered.

I understand and agree that:
- I am responsible to pay for the service call in full if the claim is denied.
- I am responsible to pay travel/trip charges for the service call.
- I may be responsible to pay for the service call at the time of service.
- I am responsible to pay for maintenance or other unapproved warranty services at $130.00 per hour labor (1 hour minimum).

Service charges may include, but are not limited to: travel, supplies, shipping, processing, maintenance, etc.

Travel charges based on distance (as the crow flies):
- $50 — 0 to 35 miles
- $100 — 35 to 75 miles
- $150 — 75 to 125 miles
- $200 — 125 to 200 miles
- $250 minimum — over 200 miles
All service calls include a $10 service supplies charge.

Ultimately, the responsibility for the bill (including all applicable charges and fees) is mine. Rates are subject to change without notice.
Please Note: A valid credit card number is required to provide service support and/or parts for your spa. We will charge your credit card $1.00 to verify your card — this amount will be deducted from your final bill. All added amendments or changes to this original authorization text are null and void.
also note that it is the spa owner's responsibility to provide hot tub service access (i.e., removal of dirt, rocks, mulch, walls, decks, railings, snow, ice, etc.).
